HealthReach Community Health Centers welcomes new provider

WATERVILLE – Dianne Raymond, PhD, PMHNP-BC will be joining HealthReach Community Health Centers in March to provide psychiatric medication management that includes psychiatric assessment, diagnosis, treatment planning, prescribing, and medication monitoring.

She will be working closely with patients and their primary care providers at several HealthReach practices to determine the best plan of care to achieve optimum wellness. Raymond will be rotating through the organization’s Belgrade, Livermore Falls, Richmond, Sheepscot, and Albion sites and will be seeing patients who are already established patients who receive medical services.

Raymond received her PhD in community mental health nursing in 1996 at University of Alabama, Birmingham and her MSN in psychiatric mental health nursing in 1987 at University of Texas, El Paso. Previously, she obtained a BA in Behavioral Science with a major in psychology at University of ME, Presque Isle. She brings psychiatric nurse practitioner experience in a variety of outpatient, inpatient and private practice settings. Her areas of expertise include adult psychiatric medication management.

“I believe that the integrated health care practice provides a forum for excellence in quality of care for patients within their community. It offers improved communication between treatment team members with increased access to medical information that serves as a basis for safe practices. Integrated primary care practices best suit my philosophy for delivery of care that addresses the whole person,” Raymond said.

HealthReach Community Health Centers is a group of 11 Federally Qualified Health Centers in central and western Maine. Dedicated providers deliver high quality medical and behavioral health care to citizens in over 80 rural communities. To ensure access for everyone, HealthReach accepts Medicare, MaineCare and major insurances. In addition, an Affordable Care Program is available to uninsured and underinsured residents as well as assistance with applications for programs that help with the cost of health care and medications including the Health Insurance Marketplace.
